Living Well with
Well with Diabetes is a program to help people with diabetes maintain and
improve their health. The program builds confidence and empowers participants
through skill building, goal-setting, problem solving, monitoring, and
reinforcement. Group problem solving is
used to help participants achieve their action plans and strategize how best to
work with their health care team and health system.
Health Services is one of several organizations in Washington State to offer
this program as part of the Group Health Research Institute Pathways to
Diabetes Self-Care Project through April of 2016.
