Many physicians are referring patients to the Group Lifestyle Balance Program. If you'd like to be referred, print the referral form here. Please have your doctor fill out the form as completely as possible and fax it to us at 509-232-8344. Alternatively you may register online, or by calling us at 509-232-8138, and bring your referral form to your first class.
Have you previously attended, or are you currently in a Group Lifestyle Balance class, but need continuing support? These accountability sessions are for you! Join us up to twice per month to weigh-in and meet with others who have also taken the class. Each month we will cover a different topic, with the main focus of the class being peer support and an opportunity to meet with a GLB coach.
The cost is $10 per month and covers both classes in a given month. You are welcome to attend two sessions per month, but we are unable to offer a discount if you are only able to attend one. Register here or download this PDF (pictured below) to view all available dates and times.
