Learn what you can do to monitor and manage your blood pressure for a healthy heart. This 4-week class series will give you the tools you need to take control of your cardiovascular health.

Topics include:

  • Healthy eating
  • How to reduce salt in your diet
  • Physical activity for heart health
  • How and when to take your own blood pressure

Participants who attend all four weeks will receive a free digital blood pressure monitor at the final class.

Many physicians are referring patients to the Blood Pressure Self-Management Program. If you'd like to be referred, print a referral form here.
